Registration Fee: Thanks to the generosity of this meeting's sponsors, the registration fee has been waived. If you have already registered, your fee will be refunded.

Important information regarding access to the Barrow Neurological Institute
Please use the entrance located on 3rd Avenue, just north of Thomas Road, to access the Barrow Lobby/Goldman Auditorium (see Campus map below,and ignore arrow pointing to front entrance/lobby).  DO NOT use the 5th Avenue entrance; it is too far away and too confusing.  If you are using the hotel shuttle, ask the driver to drop you off at this 3rd Avenue entrance.

Parking Information:
3rd Avenue Parking Garage
Parking in the 3rd Avenue garage is recommended to access the Barrow Neurological Institute and the new Leonard Goldman Auditorium.  The garage is located on the east side of 3rd Avenue, just north of Thomas. Parking is open to visitors, patients, physicians and employees. Parking is free for up to 3 hours. There is a $4.00 flat fee for parking longer than 3 hours.

Valet Parking
There is a $2.00 valet service available, located just outside the door of the lobby leading to the Goldman Auditorium, on 3rd Avenue, just north of Thomas Road.  Please note however, that this service is only available until 5:00pm.

Payment,Cancellation and Refund Policy:
You may pay for this conference in the form of a credit card or invoice guaranteed by credit card. When you visit the registration page, you will be asked to indicate your form of payment. If you request an invoice and the invoice is not paid within thirty days of the event, the credit card provided will automatically be charged for your meeting registration fee. Cancellations received on or before 5:00pm EDT on August 23, 2006 are entitled to a full conference refund. There will be no refunds after this date. If you cancel after 5:00pm EDT on August 23, 2006 and have not paid by any other means, your credit card will be charged the cost of the registration fee. If you cancel your registration after 5:00pm EDT on August 23, 2006 you may name another person from your organization to take your place.
